￥1,500 (includes ￥500 delivery fee)

*This is only the pre-registration step. You will not receive a reissue of the membership card if you do not complete the rest of the procedure. By 18 May 21:00, you should receive an e-mail from us, sent to the e-mail account you have provided on the pre-registration form. The e-mail will include the URL to a special page, where you will need to perform the actual registration.

【Please Note】
*Each TA number is limited to 1 reissued membership card.
*Please do not transfer your membership card to another person. If caught, you will be removed from the fanclub.
*This reissue is catered to members who had lost their membership cards.
*Cancellation of registration and altering of e-mail address is allowed before the closing date of 17 May noon. Please note that no changes can be made after this deadline.
*The entire reissue process may take up to 2 months.
*Payment can be made through credit card or at a convenience store.
*The reissued membership card will be sent to the mailing address provided in your fanclub account particulars. Those who have changed their address are advised to update their records beforehand on the fanclub site.

2. On the following page, key in your e-mail address, make sure the drop-down menu displays the “はい” option, then press the blue “確認する” button.
3. On the next page, press the blue button on the right, which reads “送信する”, to confirm. To make changes, press the blue button on the left, which reads “修正する”.
